I verify that the wrestler is physically able to fully participate in the Assassins Wrestling program during wrestling practice, camps, and/or competition. I acknowledge that there are risks involved in participating in sports clubs. The wrestler will make every effort to obey safety precautions and follow verbal instructions. I hereby authorize the program coaches/chaperones to act according to their best judgement during an emergency situation to include seeking medical attention for illness or injury.
I release Assassins Wrestling Club and its coaches/chaperones from any and all liability, to include financial responsibility, relating to damage, loss, illness, or injury sustained by the wrestler, regardless of whether injuries are caused by negligence. I understand that the wrestler and/or parent/guardian will assume all responsibility for any damage, illness, or injury that occurs to the wrestler because of their participation in the wrestling program and fully and forever exonerate Assassins Wrestling Club and its coaches from any and all claims, demands, damages, or causes of action, present and future, resulting from the wrestler’s participation in the club. For avoidance of doubt, participation in the program includes, but is not limited to, participating in wrestling practice, wrestling camps, weightlifting, other use/access of Assassins Wrestling Club’s facilities and equipment, interacting with other participants, transportation by the club and its coaches or chaperones, and access to off-site chaperoned facilities.
I acknowledge that Assassins Wrestling Club is not responsible for loss of personal items and understand that good judgement should be made when bringing items to the facility. As such, I further release Assassins Wrestling Club and its coaches/chaperones from any and all liability, to include financial responsibility, relating to loss or damage to personal items and understand that the wrestler and/or parent/guardian will assume all responsibility for any loss or damage to personal items occurring at Assassins Wrestling Club’s facility and/or any off-site chaperoned facilities, and fully and forever exonerate Assassins Wrestling Club and its coaches from any and all claims, demands, damages, or causes of action, present and future, resulting from loss of personal items.
I am aware that Assassins Wrestling Club runs multiple social media pages and, as a result of participation in the program, images of the wrestler may be shared on one or more of these pages or elsewhere.
I have read and fully understand and agree to the above terms.
